An ampullary adenoma was detected on EGD in this 50 year old man, endoscopic biopsies revealed high-grade dysplasia. Endoscopic ultrasound confirmed the endoscopic resectability of this tumor. The tumor was ensnared completely aiming at an Enbloc resection. A blended current is then cautiously applied until the tumor is successfully resected. Acute necrotizing pancreatitis with secondary infection of pancreatic necrosis is a highly fatal condition. Surgical necrosectomy is the treatment modality of choice with poor outcomes.
